Our Clinic Policies
Welcome to Awaken Life Centre for Fertility & Hormone Health!
Our clinic strives to provide excellent care to our clients. In order to facilitate this, we ask that you carefully review the following information regarding our clinic policies and fees if you choose to become a client of our clinic.
Any patient must complete our intake forms prior to your first official appointment. You will be asked to read and sign a separate Informed Consent form for each of the practitioners that you see at our clinic. This form must be signed before commencing any treatment.
Please note that we are NOT an IVF clinic. We do not do procedures such as IVF or IUI and we do not conduct standard fertility evaluations. For this, you must get a referral from your family doctor to a clinic such as the Ottawa Fertility Centre.
We want to be respectful of the time that each practitioner sets aside for your appointment. Last minute cancellations can be very disruptive to our schedule, and we want to be considerate to patients who are really need to be seen. As such, we have a cancellation policy in effect.
We require 2 full business days notice for cancellations. If you cancel with less than 2 full business days of an appointment, you will still be billed the FULL fee for the visit. For example, if you have an appointment on Tuesday at 10am, you must contact us no later than 10am on the Friday before the appointment.
Consideration may be given to emergencies and other unforeseen circumstances, solely at our discretion.
If you repeatedly book and cancel appointments even if it is within the allowed time for the cancellations, it is at the practitioners discretion to refuse to allow any further bookings. Please understand that it is disruptive to our schedules to have frequent cancellations, so please ensure at the time of booking that you can commit to the appointment time.

Email appointment reminders
Automated email confirmations and reminders are sent as a courtesy only.
If you do not receive a reminder email, it is still your responsibility to show up at the scheduled date and time.
Fees for our services
We are a private health care clinic, and as such, our services are NOT covered under OHIP. Each appointment will incur a fee that is to be paid at the time of the visit. If you have a private health insurance benefits plan (e.g. through work), most of our services are commonly covered up to a certain amount per year. You will have to check with your insurance provider for coverage, as each plan is unique. Please review our fee schedule here.
Confidentiality and Privacy
Confidentiality of your health information is of utmost importance to us. All staff and practitioners are trained to maintain strict confidentiality, except when sharing information is directly needed for patient/client care among practitioners of this clinic. All information obtained will be protected in accordance with the Personal Information Protection and Electronic Documents Act (PIPEDA), the Personal Health Information Protection Act (PHIPA) and that of the rules and regulations of the regulatory colleges that each practitioner belongs to.
